1.      Call Flexible Fitness Solutions to schedule Fitness by Phone®

2.      Upon receipt of payment, we will send your Fitness by Phone® Starter Kit:

            - Polar Heart Monitor

            - Caltrac Activity Monitor

- Client Fitness/Nutrition Diaries

- Training Fan

            - Health History Questionnaire and Consent

               Form

            - Schedule and Policy Agreement

            - Biography and photo of your Fitness by Phone® Coach

3.      Fax your Health History, Consent Form, and Schedule/Policy agreement   

      to Flexible Fitness no later than 3 days prior to your first phone coaching

      session.

4.      Your Fitness by Phone® coach will call you for your first scheduled 20-minute phone session and discuss how to use your heart rate monitor, Caltrac and diaries to maximize your results and motivation.

5.      Then, you and your coach will work together as a powerful team through discussion, motivation, professional evaluation and direction–along with your personal commitment-to progress you through your weekly fitness goals.
